Warrant Officer Terry L Henry was killed while piloting his aircraft on a combat operation when his aircraft was fired upon by a hostile ground force, crashed, and burned in a rice paddy about 5 miles southwest of the city of Phy Vinh, Mekong Delta, Vinh Binh Province. He was piloting a "Huey" UH-1C, Tail Number 66-15148.

Personnel in the area were on hand immediately and accounted for all personnel except Crew Chief, SP4 Stephen Harold Haight. Haight was declared dead on 7 Aug 1970 as his body was not recovered from the burned out wreckage.

On May 17, 1973, the site of the crash was again searched, aided by metal detectors, centered on the coordinates of the crash. SP4 Haight's remains again were not found. Read about the additional August 1973 recovery efforts at POW Network.
