Photo of Jack Campbell prior to his Vietnam service. It may have been taken after basic training as he is not wearing his PFC stripes or his basic ribbons. He started his tour in Vietnam on August 30, 1966. He left for Vietnam sometime after the 30th and was killed in action less than 30 days later.

Military records show that two men were killed during a contact with the North Vietnamese from small arms fire and a hand grenade exploding. They were:

PFC Jack Campbell died from metal fragment wounds to left side of the body and SFC Cristobal Melendez died of gunshot wounds to the chests and metal fragment wound to the left leg.

Jack Campbell was buried in Herrin, Illinois on Saturday, October 8, 1966.
