Jack was killed in action in an ambush near Tay Ninh. He was hit while saving another man. He received the Silver Star, Bronze Star, Purple Heart, and a Gallantry Cross with Palm and RVN National Order medals from South Vietnam. The photo below was taken at Fort Campbell, Kentucky, where Jack did his basic training, about 6 months before he was killed.
